This opportunity offers Nurse Practitioners the chance to provide comprehensive, patient-centered care within skilled nursing and assisted living environments. Schedule/Hours:
- Monday through Friday
- 8:00 AM to 5:00 PM
- No weekends or holidays
Key Responsibilities:
- Conduct thorough health assessments on residents at a minimum monthly frequency, more often as needed
- Develop and maintain individualized problem lists and medical treatment plans for each patient
- Provide primary, preventive, urgent, and acute care directly on site
- Identify and prioritize high-risk patients, creating contingency plans accordingly
- Collaborate closely with residents primary care physicians and facility staff including Directors of Nursing
- Deliver hands-on bedside care, coordinate care plans, and manage clinical orders
- Support patient transitions by assessing those newly discharged from hospitals to ensure a safe, healthy adjustment in the nursing or assisted living facility
- Manage a panel of patients across multiple facilities, typically scheduled at one site per day but flexible to visit others as patient needs arise
Qualifications:
- Master of Science in Nursing (MSN) degree
- NY Nurse Practitioner license in good standing
- Basic Life Support (BLS) certification
