This contract position offers the opportunity to provide specialized nursing care in skilled nursing and assisted living facilities with a focus on improving patient outcomes and reducing unnecessary hospitalizations. Schedule:
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Conduct rounds on patients across multiple nursing and assisted living facilities
- Perform routine monthly health assessments as delegated by the Nurse Practitioner (NP)
- Monitor residents status and support progress toward healthcare goals
- Represent the NP in meetings and communicate pertinent updates to the Interdisciplinary Care Plan (ICP) team and Interdisciplinary Care Team (ICT)
- Coordinate care and resources to enhance patient health and prevent emergency room visits or hospitalizations
Qualifications and Desired Experience:
- Registered Nurse (RN) with BSN or ASN degree
- Current New York State RN License
- Basic Life Support (BLS) certification
- Experience in long-term care nursing preferred
