What you will do:

- Manage an allocated and often complex caseload, conducting thorough assessments and formulating effective plans.
- Work closely with children, young people, families, carers, and communities to support informed decision-making and change facilitation.
- Maintain thorough and consistent case notes, reports, and records, using relevant IT systems.
- Collaborate with multi-agency teams to coordinate services and interventions, ensuring comprehensive care planning.
