Position Summary
We are seeking family medical provider who has an expansive scope of practice, which encompasses care for newborns through end of life; preferably with a focus on women’s health services that includes LARC insertion and prenatal care in low-risk patients. We are seeking a provider who is interested in working in a team-based care delivery model that integrates behavioral health services into the delivery of primary care.
Responsibilities
- Provide high quality primary healthcare services to patients of the health center utilizing a team-based model to support the Mission and Vision of HCHC.
- Utilize the health center supported electronic health record as a means of documenting and reviewing patient care services rendered or ordered.
- Contribute to the ongoing knowledge enhancement of clinical staff in order to achieve the best possible clinical outcomes for patients.
- Work to achieve the access to care, quality metric and patient satisfaction goals established by the health center.
- Participates in peer review, quality improvement initiatives, and other activities that respond to and ensure the long-term viability of HCHC.
- Provides direct clinical medical services in the area of board certified (or board eligible) medical specialty in accordance with the highest applicable standards of medical and professional practice and in full accordance with health center protocols and policies.
- Acts as a consultant on family medicine cases referred by other staff providers of HCHC.
- From time to time, provides in-service training for staff in selected topics in Family medicine as deemed necessary by the Medical Director.
- Completes medical documentation representing patient encounters within 5 business days of the patient encounter.
- Assists in internal quality improvement audits.
- Provides supervision in case assignment to Advanced Care Practitioner(s), including sign off on charts, as applicable. Applies for and obtains staff privileges at community hospitals in the area as appropriate.
- Attends internal and external meetings, workshops, and conferences, and participates on task forces or committees, as necessary and/or required by HCHC.
- Performs all duties in support of successful EHR/EPM implementation and use including Meaningful Use attestation.
- Demonstrates commitment to and understanding of HCHC's recognition as a Patient Centered Medical Home and its associated service excellence standards by modeling excellence in all clinical, administrative and team process relationships, addressing service excellence deficits as may be identified and in performance of all duties and responsibilities as a staff physician of the HCHC.
- Provides additional assistance within and outside of clinical arena, as requested and/or as appropriate, to ensure the ongoing success of the organization.
- Other duties or responsibilities as assigned by Medical Director or the Executive Director.
Required Skills and Abilities
- The provider must have an unrestricted license to practice medicine in the Commonwealth of Virginia, DEA license to prescribe medicine and board certification in chosen field of practice.
- Cannot be sanctioned under Medicaid or Medicare.
Education and Experience
- Graduate of an accredited School of Medicine
- Currently licensed by the VA State Board of Medical Examiners and Board certified DEA license with no restrictions
- 2+ years of Clinical experience in a Family Practice setting
- Working knowledge of business applications and clinical management systems (EMR)
