Physician Assistant (PA-C) – In-Home Acute Care

Make a Real Impact—One Home at a Time

At myLaurel, we are redefining acute care by bringing urgent, high-acuity, hospital-level services directly into the homes of New York’s most vulnerable, frail, and complex patients. Join a mission-driven team that focuses on patient-centered care, prevents avoidable ER visits, and reduces readmissions by nearly 50%.

About the Role

This is not a typical PA position. It’s an urgent care/hospital medicine model meeting house calls, offering the autonomy and flexibility to practice at the top of your license.

You will work as a Physician Assistant providing integrated medical services in patients' homes, primarily in the Greater NYC Area. This role is Brooklyn-focused but may include patients in Kings County, Queens County, and Nassau County.

You will travel to patients solo (but supported) in a fully stocked myLaurel company vehicle equipped with diagnostics, procedures, and medications.

  • Reports to: Director of Provider Operations

  • Partners with: Paramedics, Nurses, Physicians, and Medical Assistants

  • Employee Classification: Part-time, Hourly

Schedule and Compensation

  • Schedule: Part-Time (24 hours/week), consisting of 2 shifts of 12 hours (8:30 AM to 8:30 PM).

    • Additional shifts are available based on business needs.

    • Mandatory participation in weekend and holiday coverage is required.

  • Compensation: $85 to $95 per hour (DOE).

  • Bonus: A signing bonus is included.

Key Responsibilities

As a Field Provider, you will deliver high-quality, in-home care, focusing on urgent, problem-focused, and high-acuity situations:

  • Perform in-home clinical assessments and focused exams/health screenings.

  • Administer therapeutic interventions such as IV fluids, wound care, splinting, injections, and medication administration.

  • Order, perform, and interpret diagnostic tests, including labs, EKGs, and bedside diagnostics (e.g., POCUS).

  • Educate and empower patients and their families on health management.

  • Coordinate care across teams, contribute to care planning, and communicate with referring clinicians.

  • Practice with a palliative and patient-centered lens.

  • Ensure the timely completion of medical records.

  • Current, unrestricted New York State License as a Physician Assistant.

  • Minimum of 2+ years / 3,600 hours of experience in relevant areas (e.g., urgent care, ED, inpatient medicine, post-acute care).

  • Current BLS Certification (ACLS is preferred).

  • Active, unrestricted NYS Driver’s License.

  • Physical ability to navigate stairs and carry equipment up to 60 pounds to complete patient care in home settings.
