Job Summary
Our client is seeking a dedicated and bilingual Nurse Practitioner or Physician Assistant to join their innovative healthcare team. The primary role involves delivering comprehensive primary care services, supporting the company's mission to ensure quality and accessible healthcare for all, particularly to the Hispanic/Latino community. This position requires the ability to provide care both in-person and remotely, promote continuity of care, and contribute to a high standard of patient attention and support.
Key Responsibilities
- Provide primary care services to a diverse patient population, ensuring bilingual communication when needed.
- Work closely with Community Health Navigators to integrate clinical care with broader support services.
- Engage with patients through various communication channels, including phone and text, to ensure consistent, quality care.
- Refer patients to appropriate specialty services as needed, maintaining a focus on continuity of care.
- Promote a practice philosophy of quality over quantity, emphasizing patient-centric care over numerical targets.
Qualifications
- Licensed Nurse Practitioner or Physician Assistant.
- Bilingual in Spanish and English.
- Passionate about providing high-quality, patient-centric care.
- Committed to working with diverse populations, particularly Hispanic/Latino communities.
Skills
- Proficiency in primary care practices and procedures.
