Nurse Practitioner – Full-Time Remote

Join Virtual Physician Associates, a mission-driven telemedicine team dedicated to closing care gaps and improving health outcomes. We provide comprehensive, evidence-based virtual care to patients with chronic conditions through our innovative telehealth platform. We are seeking Family, Adult, and Adult-Gerontology Nurse Practitioners to conduct Annual Wellness Visits (AWVs) and Collaborative Care Management (COCM) visits, helping patients access high-quality care from the comfort of their homes. What You'll Do: • Conduct Annual Wellness Visits (approx. 60 minutes per patient) • Perform COCM visits (30–40 minutes per patient) • Provide episodic urgent care visits when needed • Identify and close care gaps through comprehensive assessments and follow-ups • Collaborate with patients' primary care providers to ensure coordinated care • Deliver evidence-based virtual care in a supportive team environment
