Job Title: Field Operations Specialist
Day-to-day:
- Travel to ~10 members homes a day in the Bronx and Westchester county
- Most of their business is in Westchester
- Complete at home assessment of about 6-7 questions. Should take no longer than 10 minutes. Makes sure that members are receiving the services they need and completing a home check (food in fridge, enough medication...)
- Complete data entry of records after visit
Schedule:Monday-Friday 9am-5pm
- Monday's usually in office in the Bronx to schedule visits for the week and complete any office work then Tuesday-Friday in the field
Qualifications:
- Valid NYS driver’s license and vehicle *must have a car, cannot take public transit
- Ability to visit members in their home.
- Requires a high school degree. BA/BS recommended.
- 2-3 years in a managed care or home care setting.
- Bilingual preferred – Spanish.
- MS Office required.
- Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
Responsibilities:
- Travel to member homes and conduct in-home care management visit
- Communicate effectively with members/caregivers and Plan Team (clinical and non-clinical)
- Accurate documentation and use of electronic medical record
- Timely communication and of any change to member status or environment to clinical team
- Able to manage time effectively to schedule care management visits
- Timely submission of reimbursement of travel expense
- Able to effectively read reports or manage queue to schedule care management visits
- Reasonable and predictable attendance.
- Performs other duties as assigned and directed.
- Demonstrates the ability to utilize the equipment (computer/tablet, phone) necessary to perform their job duties.
- Demonstrates ability to utilize Microsoft Office Suite
- Demonstrates the knowledge of applicable Federal, State and Local regulations as it applies to required job functions.
- Demonstrates the ability to integrate age specific requirements with individual developmental, mental, social, and cultural needs when performing procedures for geriatric residents.
- Demonstrates the ability to effectively communicate information with geriatric residents.
- Demonstrates the ability to effectively communicate information with members and their families
Key Notes:
A lot of members are elderly/have mental health issues
Background in home care industry working with nursing/insurance vendors - MLTC background
strong customer service skills
