  1. Job Description
    1. The Outreach Specialist plays a critical role in connecting individuals experiencing homelessness with essential healthcare and housing services. Working closely with hospital systems, this position involves consistent in-person outreach, collaboration with healthcare professionals, and direct support to individuals at risk of or currently experiencing homelessness. The Outreach Specialist uses a client-centered, "do whatever it takes" approach to identify needs, coordinate services, and help guide individuals toward housing stability and improved health outcomes.

 

  1. Responsibilities/ Job Duties: 
    • Conduct regular outreach at assigned hospitals and healthcare facilities to identify individuals in need of housing and support services.
    • Develop and maintain strong working relationships with discharge planning and social work teams.
    • Receive and respond to referrals from healthcare providers for patients experiencing homelessness.
    • Engage patients prior to discharge to introduce available resources and support options.
    • Assist with client enrollment into programs that provide housing navigation, case management, or other supportive services.
    • Coordinate with community providers to avoid service duplication and ensure continuity of care.
    • Provide referrals, advocacy, and linkage to services including housing, health care, benefits, and other social supports.
    • Assist clients in obtaining vital documents (e.g., ID, birth certificate, income verification).
    • Support clients in completing housing applications and navigating related paperwork.
    • Research housing availability and engage with landlords to advocate for client placements.
    • Provide crisis intervention and de-escalation support as needed in community or healthcare settings.
    • Participate in reporting and data tracking for internal and community stakeholder reporting.
  2. Minimum Education and Experience Qualification Requirements:
    • Community Health Worker Certification (or equivalent credential)
    • Valid driver’s license, reliable transportation, and current auto insurance (driving is required for this role)

Preferred Qualifications:

  • Associate's, bachelor’s, or advanced degree in social work, psychology, sociology, public health, or a related field — or equivalent lived/work experience
  • Experience using Homeless Management Information Systems (HMIS)
  • Current HMIS license
  1. Schedule/ Shift: Monday - Friday 8am - 3pm
