
Physician Assistant / Nurse Practitioner – Primary Care (Sacramento, CA – Onsite)

We are seeking a full-time, onsite Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to join a mission-driven community health center in Sacramento, CA. This is a clinic-based role (no remote or telehealth-only option), providing comprehensive primary care to diverse and underserved populations.
You will deliver evidence-based primary care, manage acute and chronic conditions, order and interpret diagnostics, prescribe medications, and develop treatment plans in collaboration with supervising physicians. You will participate in team huddles, quality initiatives, and care coordination with nurses, behavioral health, and support staff to ensure high-quality, patient-centered care.
This position is ideal for a clinician passionate about health equity and improving access to care for low-income, homeless, and medically underserved communities.
Key responsibilities
- Perform comprehensive patient evaluations, including complete histories and physical examinations
- Diagnose and manage acute and chronic medical conditions in a primary care setting
- Order, interpret, and follow up on laboratory tests, imaging studies, and other diagnostic procedures
- Prescribe medications and therapeutic interventions within scope of practice and organizational protocols
- Develop, implement, and document evidence-based treatment plans in collaboration with supervising physicians and the Medical Director
- Monitor patient progress, adjust care plans, and coordinate referrals as clinically indicated
- Initiate and manage emergency procedures when clinically indicated, including stabilizing patients and arranging higher levels of care
- Provide preventive care, health education, and counseling focused on chronic disease management and lifestyle modification
- Maintain accurate, timely, and compliant documentation in the electronic health record in accordance with agency, state, and federal standards
- Participate in care team huddles, case conferences, quality improvement initiatives, and population health activities
- Collaborate with physicians, nurses, behavioral health providers, care coordinators, and other team members to ensure coordinated, high-quality care
- Support clinic workflows and productivity standards while maintaining a high level of professionalism and customer service
Skills & Qualifications
- Active and unrestricted California Physician Assistant license (NP license also considered for Nurse Practitioners)
- Current NCCPA certification (or national NP board certification for NPs)
- Current DEA registration and BLS certification
- 1–2 years of primary care or outpatient clinical experience preferred; experience in community health, FQHCs, or with underserved populations strongly preferred
- Strong commitment to serving low-income, homeless, and medically underserved communities
- Demonstrated ability to work effectively in a fast-paced, team-based environment
- Excellent clinical judgment, communication, and documentation skills
- Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred
- Comfort using electronic health records and basic clinical information systems
Benefits
- Competitive annual salary range of $131,000 – $170,000, commensurate with experience
- Full-time schedule: Monday–Friday 8:00 a.m.–5:00 p.m. or 4x10-hour shifts (4 days/week)
